People with significant disabilities whose disability is creating a barrier toward independent living in the family, in the community or in employment.

  • Transition/Diversion Services: Assistance to those at risk for entering an institution
    • Transition out of nursing homes and other institutions to community-based living
    • Facilitate the transition of youth with disabilities to higher education and /or the workforce and the community
