- Transition/Diversion Services: Assistance to those at risk for entering an institution
- Transition out of nursing homes and other institutions to community-based living
- Facilitate the transition of youth with disabilities to higher education and /or the workforce and the community

- Americans with Disabilities Act Technical Assistance
